Blue Cross and Blue Shield of NC - eligible for 6 free visits (Blue Care HMO, Blue Options or Blue Advantage) if a member of the Healthy Lifestyle Choices Program (enroll by calling 1.800.218.5295).

Members of the following insurance companies are automatically eligible for a discounted fee schedule of $75.00 for new patient and $50.00 for established patient office visits (payment to be made at time services are rendered):

Coverage and conditions of payment vary by plan and group contract (refer to your specific contract for the coverage, deductibles, coinsurance and other conditions of payment). Please bring your insurance card with you to each visit.
